SUMMARY:
Responsible for implementing, monitoring, and on-going assessment of the housing and social service plan for all families assigned to the City Sanctuary Program.
RESPONSIBILITIES:
- Completes family intake, comprehensive family history and maintains accurate and timely family case records.
- Responsible for designing social service plan in cooperation with new residents.
- Secures entitlements for clients, serves as advocate for families in case of conflict.
- Responsible for advocacy and providing assistance to clients in obtaining permanent housing.
- Make referrals to special housing agencies.
- Meet with clients to develop strategies to meet their housing needs.
- Coordinate permanent housing search.
- Escort families to view apartments and meet prospective landlords and real estate brokers.
- Makes referrals for additional services such as mental health, and/or medical services.
- Serves as a liaison with daycare and schools to monitor admission and attendance of client’s children.
- Follows up with open child welfare cases, open court cases, and parole/police matters.
- Works collaboratively with Shift Supervisor and Resident Aides.
- Participate and attend the Interdisciplinary Team and Administrative meetings weekly.
- Complete / enter client case management record in DHS-CARES system
Qualifications
The position requires an HS Diploma/GED and comparable experience. BA preferred. Previous experience with the homeless population is needed. Needs the ability to establish workload priorities and balance diverse caseloads. Must have exceptional communication skills both orally and in writing with individuals and groups regarding complex and sensitive issues or regulations. Bilingual Spanish/English is helpful. Physical ability to perform the essential job functions is needed. In addition, the Case Manager shall perform any duties as required to meet the needs of the program and the clients that we serve.
