Job Description
License Practical Nurse
Summary: Responsible for providing medical support to Day and Residential programs by performing the following duties.
Essential Duties and Responsibilities
1. Complete monthly review and quarterly assessments for individuals on their caseload.
2. Stock med rooms in home weekly and reorders supplies as needed.
3. Work with the psychiatrist monthly; takes orders, documents, etc.
4. Work with pharmacy to reorder medications, package medications for home visits, reorder medication as needed, assists pharmacy to stock meds monthly, transfers MARs monthly, etc.
5. Follow up on incident reports as needed and maintains an incident report log.
6. Communicate report to administrative staff during the week.
7. Represent the nursing department at the monthly safety committee meeting.
8. Complete wellness monitoring exams with D.O.N. oversight.
9. Administer medications and completes treatments as directed.
10. On-call weekends on a rotating basis.
11. All other duties as assigned.
Education and/or Experience:
Graduate from an accredited nursing program. Prefer 6-12 months experience with individuals with developmental disabilities.
Certificates, Licenses or Registrations:
Current practical nurse license. If not licensed in the state you are applying for be willing to obtain within six months of hire. First Aid and CPR certified. Must have valid driver’s license and driving record that meets Project ReDirect standards.

Physical Demands:
Must be able to sit at computer keyboard and input data for extended periods of time. Must be able to walk, bend and stand frequently.
