Case Manager
Job Description
Duties:
- Provide case management services to adults that are unsheltered or at risk of becoming unsheltered, justice involved/impacted, drug using community, behavioral health impacted,, and other vulnerable adults in need
-Provide non-judgmental care to the individuals served
- Conduct needs assessments to determine client needs and develop appropriate service plans
- Coordinate and monitor the delivery of services to clients
- Collaborate and support clients in achieving their goals
- Utilize motivational interviewing techniques and strength based care to engage and empower clients
- Collaborate with community partners and agencies to ensure comprehensive support for clients
- Advocate for clients' rights and access to services
- Maintain accurate and up-to-date case records
Experience:
- Previous experience working with Justice involved, behavioral health, unsheltered, , or related fields is preferred
- Previous lived or living experience is preferred
- Knowledge of Harm Reduction, Med-Quest, community resources, housing voucher systems, and other government assistance programs is a plus
- Strong communication and interpersonal skills
- Ability to work effectively with diverse populations
- Excellent organizational and time management skills
- Ability to work independently and as part of a team
