- Comprehensively assess patient goals, including biophysical, psychosocial, environmental, and discharge planning needs, as well as financial status.
- Closely monitor patient progress to identify changes in medical needs that may affect present and future treatment.
- Procure services and act as an advocate on behalf of the patient and family to secure scarce resources.
- Collaborate with third-party payers to resolve issues affecting patient care continuity.
- Coordinate and direct patient discharge planning with family members, healthcare teams, and the community.
- Review admission data within designated timeframes to determine suitability of care levels according to established criteria.
Skills:
- Familiarity with nursing practices and procedures.
- Strong clinical assessment skills.
- Knowledge of community resources available to facilitate patient recovery.
- Understanding of third-party payer issues related to reimbursement and regulatory guidelines.
- Ability to explain complex information in a clear, understandable manner.
Requirements:
- Graduation from an accredited school of nursing.
