Travel Nurse RN Case Management

We are seeking a skilled Travel Nurse RN Case Manager to join our team. This 13-week assignment offers a unique opportunity to make a meaningful impact in the healthcare industry.

Key Responsibilities:

• Comprehensively assess patients' goals, including biophysical, psychosocial, environmental, discharge planning needs, and financial status.
• Closely monitor patients' progress for changes in medical needs that could affect present and future medical treatment.
• Procure services and serve as an advocate on behalf of the patient and family with scarce resources.
• Work closely with third-party payers to resolve problems and issues affecting the continuity of patient care.
• Coordinate and direct the discharge planning of patients with family members, healthcare teams, and the community.

Requirements:

• Graduate from an accredited school of nursing required.
• Strong clinical assessment skills and knowledge of nursing practices and procedures.
• Must be knowledgeable about resources available in the community for use by patients, families, and the healthcare team.
• Requires knowledge of third-party payer issues concerning reimbursement and regulatory perimeters.
