This is a professional-level position for a Case Planner, specializing in family support services. This position is ideal for individuals with substantial experience in case planning and management, particularly in aiding at-risk families. This role is pivotal in delivering high-quality, compassionate, and effective services to families in need. The ideal candidate will be adept at managing multiple responsibilities and will have a proven track record of working effectively with diverse populations, particularly those at risk.

KEY ESSENTIAL FUNCTIONS:
- Expertise in Case Management: Demonstrating significant knowledge and experience in case planning, particularly for at-risk family groups.
- Effective Communication Skills: Possessing strong listening and communication abilities, essential for accurately gathering, interpreting, and conveying information both verbally and in writing.
- Non-Clinical Counseling and Advocacy: Providing essential support services to clients and their families. This includes but is not limited to, translation and interpretation services, acting as a parent advocate in Family Court, coordinating transportation, and facilitating linkages and referrals to both in-house agency resources and external service providers.
- Information Management: Responsible for the diligent maintenance and sharing of client information with the ECDSS worker through the Citrix Connections computer program. This also includes managing files for each client within the Preventive Services Program.
- Bilingual Proficiency: The position demands strong communication and written skills in both Spanish and English, catering to a diverse client base.
REQUIREMENTS:
- An associate degree (bachelor’s degree preferred) from an accredited institution in vocational rehabilitation, education, social work, human resources, public policy, social sciences, or a related field is preferred, and at least one year of paid experience in human services. And
- Bilingual and bicultural and fluent communication skills in both Spanish and English a plus. Must be able to facilitate a good working relationship with county caseworkers and be knowledgeable of and sensitive to the special needs' population.
- Must have a valid driver’s license and a reliable and fully insured vehicle.
