POSITION OVERVIEW:
The Health Care Coordinator is responsible for the overall day to day coordination of medical appointments of all individuals who reside in the residence and must follow all procedures in accordance with the agency, state and federal regulations. The Assistant Manager is also responsible for the direct supervision of the direct service providers. The Assistant Manager assures that all the individuals receive quality care and required supervision in reference to the residential habilitation and oversight plans in addition to effective implementation of the program plans for all individuals. The Assistant Manager ensures the program is operating with all Acacia Network expectations, including programmatic, billing, financial, facilities, and quality. The assistant Manager is available on call 24 hours, 7 days a week.
KEY ESSENTIAL FUNCTIONS:
- Monitor individual’s health through coordination with Residential managers’ and nurse.
- Schedule medical/dental/therapy appointments accordingly.
- Document requests of healthcare received.
- Maintain preventive healthcare records for each of the individuals to ensure they receive recommended tests and procedures relative to their age, medication and medical history.
- Maintain medical records in compliance with OPWDD standards and updated annually.
- Ensure clients’ healthcare needs are following OPWDD standards and updated annually.
- Oversee prescription ordering and refills.
- Provide direct support to clients as needed.
- Provide transportation for clients to various appointments and other destinations daily.
- Serve as a positive role model for Individuals.
- Must successfully complete all required trainings.
- Communicate all relevant information to all residential staff and supervisors verbally and in writing for the purpose of providing doctors instruction, providing feedback, and adjusting medical practices as per physician orders.
- Always maintain HIPPA practices & confidentiality of all individuals.
- Perform other duties as required.
REQUIREMENTS:
- Minimum High school Diploma.
- Experience providing services to developmentally disabled adults.
- Training in First Aid and CPR, general health and familiarity with diagnosis of various developmental disabilities.
- Knowledge of OPWDD medical regulations.
- Possession of Valid NYS Driving License and the ability to transport residents in an agency vehicle.
- Ability to work with minimal amount of supervision.
- Effective oral and written communication skills.
- Effective organization skills.
