Job Overview
We are seeking a dedicated and experienced Field Supervisor to over see the delivery of home health care services to clients in their homes, ensuring that caregivers adhere to established care plans, comply with regulations, and provide quality care by actively monitoring their performance, conducting client assessments, and providing training and support to the field staff.
Duties
- Client Assessments and Care Plan Development: Conducting initial and ongoing client assessments to evaluate needs, developing individualized care plans, and making necessary adjustments based on client changes.
- Caregiver Supervision: Monitoring the performance of home health aides and other caregivers to ensure they are following care plans, providing appropriate care, and adhering to company policies and regulations.
- Staff Training and Development: Providing ongoing training to caregivers on new procedures, best practices, and compliance requirements, including orientation for new hires.
- Communication and Collaboration: Maintaining open communication with clients, families, physicians, and other healthcare professionals involved in the client's care.
- Incident Reporting and Management: Investigating and documenting any incidents or changes in client condition, taking corrective actions as needed.
- Documentation and Recordkeeping: Maintaining accurate and complete client records, including progress notes, care plan updates, and incident reports.
Skills
- Experience in home health care: Prior experience providing home health services is essential.
- Bilingual: English and Spanish
- Excellent communication skills: Strong verbal and written communication skills to effectively interact with clients, families, and healthcare providers.
- Leadership and supervisory skills: Ability to lead and motivate a team of caregivers.
- Clinical assessment skills: Ability to accurately assess client needs and develop appropriate care plans.
- Problem-solving abilities: Ability to identify and address issues that may arise in the field.
- Computer proficiency: Familiarity with electronic health records and other software applications used in home health care.
