We are seeking an experienced Registered Nurse (RN) to provide exceptional care within a long-term care/skilled nursing facility. The ideal candidate will ensure health maintenance, illness prevention, and patient advocacy. This role includes medication administration, health assessments, and responding to behavioral or medical crises while maintaining compliance with nursing standards and regulations.

Responsibilities
- Provide nursing care and oversight, including health assessments to promote wellness and prevent illness
- Administer medications and treatments per established procedures
- Collect and document health status data, ensuring accurate and accessible medical records
- Identify and respond to acute health problems by initiating interventions or referrals, including follow-up care
- Serve as a first responder for behavioral or medical crises, including emergency situations involving severe or violent behavior
- Maintain and update individual records and perform data entry of medical information
- 2 years of experience in long-term care or skilled nursing
- Completed Dementia Training certification or equivalent
- Availability to work every other weekend, rotating days off, and all holidays as required
- Active RN License in MA
