Position : Social Worker (LBSW)
Pay Rate : $58.00 / hr Location : Houston, TX (LTC / Skilled Nursing Setting)
Job Summary :
Protouch Staffing is looking for a dedicated Social Worker to join our team for a 13-week contract position. In this role, you will support patients with complex psychosocial needs, providing assistance with eligibility determination for social programs, funding sources, and accessing community assistance. Additionally, you will offer crisis intervention, facilitate discharge planning, and coordinate care for high-risk patient populations.
You will collaborate with an interdisciplinary team-including physicians, case managers, staff nurses, and other healthcare professionals-to provide comprehensive support for at-risk patients, ensuring that all psychosocial needs are addressed throughout their care.
Key Responsibilities :
Intervene with patients who have complex psychosocial needs, helping them navigate social programs and funding opportunities.
Offer crisis intervention services to patients and families as necessary.
Develop and coordinate discharge plans for high-risk patient populations, ensuring continuity of care.
Work closely with an interdisciplinary team, receiving referrals for at-risk patients from physicians, case managers, nurses, and other healthcare team members.
Identify and connect patients with appropriate community resources to improve their care and outcomes.
Provide psychosocial and therapeutic counseling, supporting patients and families through complex situations.
Minimum Qualifications :
Bachelor's degree in Social Work.
Licensed Baccalaureate Social Worker (LBSW) and licensure as a social worker in the state of Texas (required).
Experience / Knowledge / Skills :
Three (3) years of experience in Long-Term Care (LTC) or Skilled Nursing.
Strong working knowledge and / or experience in utilization management , managed care , and payer issues .
Experience in psychosocial and therapeutic counseling .
Exceptional interview , assessment , and organizational skills with the ability to problem-solve in complex situations.
Ability to identify and connect patients with the appropriate community resources .
Proven ability to collaborate effectively with patients , families , healthcare teams , and community agencies to achieve optimal patient outcomes.
Strong interpersonal communication , negotiation , and analytical skills .
Experience or exposure to pre-acute and post-acute care as well as community resources.
