Hospice Admissions RN (Part Time)

Description

This position will be working part time (24 hours / week) on evening shift (Sundays and Mondays are required days) and traveling to see patients in Orange county.

The Admissions RN implements programs to inform clients of St. Joseph Health, Home Care Services (HCS) capabilities and benefits. Coordinates home health services after patients are referred to assist in the transition from hospital to home and ensure continuity of care between hospital or clinic setting and home.

Required Qualifications :

• Education sufficient for licensure.
• upon hire : California Registered Nurse License.
• Driving is a necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
• 1 year Registered Nurse experience.
