SUPERVISION: Assistant Program Director
JOB DESCRIPTION:
The Social Worker under the direction of the Assistant Program Director is responsible for ongoing needs assessment, individual treatment plans, implementation, support, information, referral and advocacy for individuals who are in the Providence House
Supportive Housing facility. Residents in our Supportive Housing Program are individuals living with mental illness, and/or have a history of substance abuse as well as homelessness. The position requires successful working relationships with residents that support their ability to increase their self-reliance and independent living skills. The Social Worker will be responsible for influencing and recognizing positive behavior changes and identifying problems and obstacles that threaten to undermine the resident’s goals. Keeping clear and accurate files and records is essential to this position. The position will require occasional evening and weekend work hours.
REPORTS TO: Assistant Program Director
RESPONSIBILIITES:
Client/Service Delivery
- Maintain a minimum caseload of 12 families and/or individuals. Meet weekly with residents to develop and monitor each individual’s service plans;
- Complete strength-based comprehensive needs assessment that will inform individualized service plan;
- Facilitate groups around topics such as health and wellness;
- Provide client-centered services that support independent living skills;
- Provide crisis management and intervention;
- Inspect resident apartments to ensure that they are properly maintained;
- Be knowledgeable of community resources and refer clients for services needed;
- Organize and facilitate social and recreational activities designed to promote community involvement and peer support;
- Work with the Mental Health Clinician and Assistant Program Director to ensure that residents are linked with services such as mental health, substance use, education and employment;
- Attend and utilize trainings that increase knowledge of effectively meeting client’s needs through evidence-based practices;
- Work with all members of the household, including children, to ensure that each are meeting their full potential;
- Assess client mental health and recovery needs;
- Conduct Psychosocial and other assessments;
- Work with children within the household to ensure that their medical, educational and other needs are being met;
- Performs other duties as assigned by the Assistant Program Director.
Sanctuary Model
- Effectively use and practice Sanctuary Models tools; safety plans, community meetings, psycho-educational groups, team meetings, red flags reviews, and self-care in the program to create a non-violent and trauma sensitive culture.
- Promote and model the core values of Providence House Mission (Compassion, Non-Violence, Hospitality, and Community) and the Seven Sanctuary Model Commitments to create a non-violent and trauma sensitive culture.
- Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment.
Team/Staff Relations
- Work as a Providence House team member with all staff and volunteers in order to implement the mission and core values of the agency.
- Attend regular internal and external agency meetings.
- Work as part of team to plan and develop resident programs.
Administrative
- Keep accurate, up-to-date documentation on all clients. Including but not limited to, treatment plans, case notes assessments, etc.
- Complete all paperwork in a timely and accurate manner that ensures compliance with stakeholders.
- Assists with any and all data collection systems.
REQUIREMENTS
- Master’s Degree in Social Work or related field required.
- Two years of experience working with the formerly homeless, mentally ill population preferred.
- Experience with long-term homeless, low income and diverse populations.
- Have working knowledge of major psychiatric disorders and substance use disorder.
- Excellent verbal and written communication skills and the ability to work independently.
- Competent technology management skills, including proficiency with using Microsoft Office; client database management.
WORK SCHEDULE
The Social Worker position is a full time, salaried position. Hours worked will include one late evening weekly and a weekend day twice monthly. The office location for this position in Brooklyn, New York.
SALARY AND BENEFITS
Salary range $56,000 - $60,000 Annually. Paid time off benefits include: 14 paid holidays, 3 weeks of vacation, 3 personal days, 12 sick days, early closure proceeding holidays, and a day off annually in recognition of your Birthday.
