Description
The ALS RN Coordinator is responsible for monitoring medical, physical, and psychosocial care needs of patients receiving care from the Providence Amyotrophic Lateral Sclerosis (ALS) Center. The RN provides direct patient care and case coordination from diagnosis to end of life.

Required Qualifications :
- Bachelor's Degree - Nursing; from an accredited RN School of Nursing.
- National Provider BLS - American Heart Association upon hire.
- Oregon Registered Nurse License upon hire.
- 2 years - Nursing experience working with patients who have neurological conditions or ambulatory care, home health, acute care and / or case management.
- Background in telephone triage, outpatient clinic, ambulatory care, home health, acute care, hospice, and / or case management.
- Experience with an electronic medical record system.
