Join our team as a Clinical Program Coordinator for the Amyotrophic Lateral Sclerosis (ALS) Center, where you'll play a vital role in monitoring the medical, physical, and psychosocial needs of patients. As an RN Coordinator, you will offer direct patient care and comprehensive case coordination from diagnosis through end-of-life care, ensuring that our patients receive the best possible support.

Required Qualifications :
- Bachelor's Degree in Nursing from an accredited RN School of Nursing.
- National Provider BLS certification from the American Heart Association upon hire.
- Oregon Registered Nurse License upon hire.
- At least 2 years of nursing experience with patients who have neurological conditions or in ambulatory care, home health, acute care, and / or case management.
- Experience in telephone triage, outpatient clinic, ambulatory care, home health, acute care, hospice, and / or case management settings.
- Familiarity with electronic medical record systems.
