Description
This position involves traveling to see patients primarily in the Portland area.
The Hospice Nurse, under physician direction, completes medical requirements while coordinating a whole person plan of care. The position is accountable for direct and indirect quality care for patients of all ages. Care is provided according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. The position accepts professional development as an integral part of nursing practice. It coordinates patient care activities with other members of the healthcare team in a collegial relationship. Confidentiality, attendance, and punctuality are upheld.

Required Qualifications:
- Education meeting certification, license, or registration requirements.
- Oregon Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- Washington Registered Nurse License upon hire or obtain within 90 days of hire.
- Two years of Medical/surgical experience in an acute care setting, and/or a combination of medical/surgical, long-term care, or home health/hospice experience.
- Experience using electronic medical record software.
