RN - PACE/Elderplace at Providence
As a Community Care RN, I provide and supervise professional nursing care based on the nursing process, ensuring optimal health and independence for elderly patients. My role requires a comprehensive understanding of nursing practices and the ability to work in various settings to empower older adults to live active lives. I also collaborate with a multidisciplinary team to deliver exceptional healthcare services in accordance with the Oregon Nurse Practice Act.
Description
This is a full-time position supporting the PACE Cully site.
The Community Care RN provides and supervises professional, independent nursing care for Providence patients. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. Applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients' optimal health and independence.
Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits, and more.

Required Qualifications:
- Coursework/Training: Relevant training applicable to the position, as specified by the Nurse Manager
- Oregon Registered Nurse License upon hire
- within 30 days of hire: National Provider BLS - American Heart Association
- Upon hire: Relevant certifications applicable to the position, as specified by the Nurse Manager
Preferred Qualifications:
- Bachelor's Degree in Nursing.
- upon hire: Current recognition by a national professional organization in care management, geriatrics or hospice
