Responsibilities
As a skilled nursing care professional, your responsibilities will include:
- Developing, implementing, and evaluating individualized patient care plans in collaboration with the interdisciplinary team.
- Providing comprehensive assessments, evaluations, and interventions to ensure optimal patient outcomes.
- Collaborating with other healthcare professionals to coordinate care and services.
Requirements
Qualified candidates must possess:
- A minimum of two (2) years of RN experience in home health or a related field.
- Valid driver's license and reliable transportation.
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
- Current CPR certification.
