JOB OVERVIEW:
The Care Coordination Data Specialist is responsible for providing the medical data needed for coordination of care activities to Ryan White HIV Medical Case Management (MCM) providers in the Philadelphia Eligible Metropolitan Area (EMA). This employee will review incoming medical data requests, maintain signed consent forms, extract requested information from an internal database, and transmit data to providers through a secure FTP site. Additionally, this employee will complete daily data entry into the CAREWare database, and complete monthly data imports into Microsoft Access. This role requires strong attention to detail, the ability to handle protected health information with the utmost integrity while adhering to all applicable federal, state and local laws and regulations, and the ability to plan and organize work to complete assignments accurately within established time frames.
This full-time position reports to the Quality Management (QM) Coordinator and is part of the QM Team located in the Information Services Unit (ISU) of the Philadelphia Department of Public Health’s Division of HIV Health (DHH).

RESPONSIBILITIES:
- Review incoming medical data requests for completion and compare against signed consent forms.
- Maintain records of signed consent forms.
- Extract requested information from an internal database and transmit to MCM providers through a secure FTP site.
- Import clinical data updates from funded medical providers into the internal database on a monthly basis.
- Work with staff in Client Services Unit to provide medical data needed for new client intakes on a daily basis.
- Enter demographic and service information related to new client intakes into the CAREWare data system.
- Other duties as assigned.
- Strong organizational skills
- Effective communication (written and verbal) and interpersonal skills
- Proficient with MS Word, PowerPoint and Excel
- Demonstrated ability to work independently and as part of a team
- Experience navigating Microsoft Access preferred
- Experience working with protected health information preferred
