Job Description
The PHBPP Coordinator will be housed within the DDC Viral Hepatitis Program and work to meet the nationally established program standards as well as local goals and initiatives. The person in this role will work closely with other Viral Hepatitis Program staff along with the DDC Immunization Program, Acute Communicable Disease Program, Epidemiology, health care providers and hospital networks, and CDC and other jurisdictional PHBPP Project Coordinators. They will be supervised directly by the Viral Hepatitis Program Manager.
The applicant must work from the Philadelphia Department of Public Health offices or other locations as assigned. This job may require moderate physical effort including lifting materials and equipment of less than 50 pounds and involves viewing a CRT or VDT screen 50 to 75 percent of the time. Position duties are frequently performed in the field, at various locations across the city. The candidate must have access to a personal vehicle, or be willing to use public transportation to reach assignment sites, or use other mode of transportation satisfactory to performing required field work. - RESPONSIBILITIES:
- Functions as the principal coordinator of activities for PHBPP within the Viral Hepatitis Program, in close collaboration with staff from allied DDC Programs – including Immunization, Acute Communicable Disease Control, and Epidemiology.
- Responsible for implementation of PHBPP goals and objectives regarding PHBPP disease surveillance, case management, control, and prevention of perinatally acquired hepatitis B.
- Coordinates case management, education, and support for Philadelphia residents living with Hepatitis B who are supported by the PHBPP
.·Retains working knowledge of up-to-date Hepatitis B clinical, surveillance, and perinatal Hepatitis B literature and public health practices.
- Actively educates healthcare providers about the epidemiology, and clinical care for perinatal hepatitis B and hepatitis B infections in pregnant and postnatal people.
- Spearheads programming and quality improvement activities to reduce perinatal Hepatitis B transmission and improve outcomes for parents living with Hepatitis B in alignment with local and national elimination goals.
- Develops and maintains consistent and effective relationships with the city’s hospitals and health care providers to ensure that pregnant parents with hepatitis B disease are identified and managed, and that their infants receive appropriate testing and vaccination at birth and during infancy to prevent chronic hepatitis B infection.
- Functions as a liaison with the CDC’s National Centers for Immunization and Respiratory Diseases to coordinate program planning, operations, reporting and evaluation.
- Prepares and submits monthly, quarterly, semi-annual, and annual reports as required by CDC and other governmental authorities, as well as ad hoc reports requested by the Viral Hepatitis Program Manager.
- Performs programming- and outcomes-related dissemination activities at local, state, and national level.
- Collaborates with Viral Hepatitis staff on activities supporting Philadelphia residents living with hepatitis B and C.
- Support Philadelphia Hepatitis B & C Elimination plan activities, especially as they relate to hepatitis B.
- Participate in emergency preparedness exercises and real response activities at the discretion of the Viral Hepatitis Manager and DDC Division Director
- Plan, implement, manage, and evaluate public health programs.
- Perform public health surveillance of a vaccine-preventable disease.
- Independently develop proposals for grant funding, including the establishment of program standards and objectives and reporting of program outcomes and statistics.
- Effectively communicate with PDPH staff, health care providers, and partners allied in assuring identification, testing and vaccination of persons at risk for Hepatitis B.
- SKILLS:
- Ability to convey program subject matter and education at a variety of levels in a professional manner – among professional health care providers and community residents.
·Thoroughly understand and effectively navigate health care delivery system. ·Knowledge of and ability to work and engage with individuals who are part of specific populations, including incarcerated individuals, persons who use drugs, immigrants, uninsured, under-served and under-represented populations.
- Working knowledge of hepatitis B, including transmission, prevention strategies, laboratory and treatment information preferred (additional training will be provided).
- Strong written and verbal communications skills.
- Must be skilled in Microsoft Office suite and have the ability to manage timely and thorough data collection.
- Able to handle multiple tasks and support project timeline with a high degree of organizational skill.
- Able to work effectively and professionally in a fast-paced environment
- EXPERIENCE:
- Minimum of 3 years of experience working in skilled positions in Public Health Programs.
- Case management of patients living with an infectious disease, preferred.
- EDUCATION REQUIREMENT:
- Bachelor’s Degree in public health or related field. Clinical degree preferred.
- Relevant years of experience can be substituted for bachelor’s degree
