Job Overview:

The SAFE Program Coordinator (PC) is a part-time position that supports the SafeCare Augmented for Family Empowerment (SAFE) program. SafeCare Augmented, is an evidence-based home visiting service (EBHV), with the purpose of empowering families, supporting parents, and promoting children's health, development, and safety for Philadelphia families. SAFE PC will support this program through program coordination, intake, and community health navigation services. The SAFE PC will provide health-focused home visiting/video based/telephonic services to families with children 0 – 5years of age. Primary responsibilities include: coordinating program activities, supporting staff; conducting intakes with clients, supporting the development of project reports and deliverables, data base management, general support and assistance to Program Manager of SAFE, and provide direct services to clients. The SAFE PC will also be responsible for communicating directly with the funder, Office of Child Development and Early Learning (OCDEL). This position will have both internal project coordination responsibilities, as well as outward-facing responsibilities. As such, this position requires strong project and time management skills, as well as strong oral and written communication skills. The SAFE PC must be willing to work a flexible schedule including some evening hours, as requested.

Responsibilities:

Administrative/Data Management Activities (Primary Responsibility)

  • Coordinate program activities, including supporting Program Manager for SAFE program and direct support to staff and staff activities.
  • Support Program Manager for SAFE program with quarterly, monthly, and various reports to funders and HPC leadership.
  • Attend program meetings with funders.
  • Maintain and update tracking on database and in Excel on the department shared drive.
  • Maintain data entry and monitoring of data in accordance with relevant protocols.
  • Communicate regularly to OCDEL, SAFE Program Manager and program staff on data and evaluation collection and entry status, anticipate challenges, and offer recommendations when appropriate.
  • Maintain schedule for data collection, entry, cleaning, and reporting.
  • Develop a professional/outreach network of healthcare, school, and parenting contacts.
  • Document program activities in accordance with guidelines/protocol to support strong project data collection and/or publication opportunities.
  • Support Program Manager for SAFE program in oversight of material and supply inventory.
  • Communicate formally on a bi-weekly or weekly basis with Program Manager for SAFE.
  • Attend local and regional meetings and trainings, as needed.
  • Identify opportunities for program expansion.
  • Support SAFE staff to develop recruitment and referral strategies and assist Program Manager for SAFE with development of plan to meet targets.
  • Give presentations on the program, as requested, to parent and community groups, school boards, and funders.
  • Other relevant duties as assigned.


Direct Service Delivery (Secondary responsibility)

  • Support other navigators by delivering SAFE services to parents in Home or Virtually for a limited amount of clients.
  • Maintain a point of contact with referral sources and potential clients to services.
  • Provide and monitor services to families as outlined in the SAFE Model Implementation Training.
  • Complete relevant SAFE surveys/assessments with caregivers and assist staff in completing surveys, as needed.
  • Provide incentives, education, referrals, and linkages for participant to services, including health insurance, home health care, educational system, mental health, and various other needs.
  • Maintain a calendar for intakes to streamline processes and enhance efficiency to complete intakes with referred clients.
  • Serve as a liaison with appropriate parenting programs, community-based organizations, school districts staff, and other early childhood programs.
  • Prepare for incentive drop-offs including coordinating with administrative assistant to order supplies and materials.
  • Serve as liaison between community contacts and partners and HPC management team, providing updates on an ongoing basis in written and verbal formats.
  • Provide individualized education, promote health literacy, and provide community health navigation services for individuals and families in Philadelphia County.
  • Participate in case conferences and required/necessary trainings.

  • Excellent written and oral skills
  • Must demonstrate strong organizational, time management and problem solving skills
  • Comfortable delivering interventions to families in various home situations
  • Open and responsive to coaching and constructive feedback
  • Understands the importance of program fidelity
  • Ability to assess and triage social services quickly
  • Ability to advocate for client and community strengths and needs
  • Advanced proficiency in Microsoft office suite and various web-based platforms with an ability to learn new software, as needed
  • Must clear child abuse, criminal history check and FBI clearance
  • Must travel in Pennsylvania
  • Ability to work both independently and in a team environment to meet objectives with minimal supervision
  • Ability to independently lead a small team
  • Ability to acquire information about new systems, organizations, and practices
  • Motivational interviewing techniques
  • Must have ability to work outside “normal business hours”; evenings and weekends are required


Preferred

  • Excellent understanding of the community-based organizations throughout Pennsylvania


Experience:

  • Demonstrated experience in issues related to child development and safety
  • At least two years' experience in community health and home visits preferred
  • Experience successfully coordinating community events with multiple stakeholders
  • Experience in data collection/entry and evaluation monitoring
  • At least three years experience working with diverse populations, low-income individuals, individuals from different backgrounds
  • Minimum of three years of direct in-home, community-based and/or clinical services
