PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.

New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.

This is a grant-funded position ending March 31, 2027.

Program Description :

The Healthcare-Community Partnerships (HCP) Capacity Building team applies best practices and innovative methods from PHS’ experience in sexual and reproductive health and healthcare-community partnerships to the execution of scalable healthcare capacity building projects relevant to the latest healthcare reforms that attempt to improve health access and equity in New York. The HCP Capacity Building team will support the trustworthy integration of health-related social needs (HRSN) screening and referral to community resources into a broad range of healthcare practice settings within PHS’ SCN, including hospital care, primary care, sexual and reproductive healthcare, behavioral health, and care management programs.

Position Summary :

We seek an experienced Practice Improvement (PI) Specialist who will lead quality improvement (QI) and capacity building activities for one or more Quality Improvement Learning Collaboratives (QILCs) focused on supporting healthcare partners to implement HRSN screening using a required tool, refer to PHS’ SCN, and navigate clients to ensure identified HRSN needs are met. QILC activities to be led by the PI Specialist include QI coaching, technical assistance (TA) and training. The ideal candidate will have a strong background in public health and QI, excellent communication skills, and a passion for fostering collaborative partnerships to improve health outcomes.

The PI Specialist will report to one of the following members of the team : Director, HCP-CB; QI and Evaluation Manager; or Training Manager and will work in close collaboration with the rest of the HCP Capacity Building team.

Specifically, the PI Specialist will :

  • Provide direct TA to healthcare partners, including individualized support, improvement coaching and resource development. Maintain accurate record of TA activities, including attendance and meetings minutes.
  • Support the development of QI materials and tools (e.g., change packages, improvement plans, self-assessments, knowledge / attitude / practice surveys, quality measures, toolkits, etc.) to enhance healthcare partner capacity for HRSN screening, referral and navigation.
  • Manage the ongoing completion and review of QI materials and tools by healthcare partners.
  • Analyze and interpret quality measures data to assess progress towards improvement goals and facilitate next steps for improvement in coordination with the QI and Evaluation Manager.
  • Lead learning session content development and facilitate sessions as appropriate, in collaboration with Training Manager, Program Manager, and QI and Evaluation Manager Support Training Manager to identify training needs and craft training offerings.
  • In collaboration with the Training Manager, facilitate trainings related to HRSN screening, referral, and navigation
  • Contribute to design of QILCs and broader QI strategy discussions, including ways to support participants to share successful improvement practices and facilitate collaborative learning.
  • Build and maintain strong relationships with healthcare partner improvement teams
  • Prepare reports, presentations, and other documentation, as required.

Qualifications and Experience :

  • Bachelor’s degree or equivalent in Public Health or related field; Master’s degree preferred.
  • Minimum 2-3 years of experience in QI, capacity building, technical assistance, or a related discipline.
  • Direct experience supporting continuous quality improvement processes with health care facilities strongly preferred.
  • Foundational knowledge of community, city and state health department systems and NYC-based provider networks. Prior experience in issues related to healthcare; social services; health equity, access and quality; population health (or some subset / combination of these) preferred.
  • Salary : $72,000 - $72,000
  • Desired Skills :

  • Excellent verbal and written communication skills, with the ability to convey complex information clearly and effectively.
  • Strong interpersonal skills. Demonstrated ability to build and maintain collaborative relationships with diverse stakeholders.
  • Ability to work independently and as part of a team, managing multiple priorities and deadlines.
  • Highly organized and detail-oriented with exceptional time and project management skills.
  • Proficiency in Microsoft Word, Excel, PowerPoint and use of web-based meeting applications.
