PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.
New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.
This is a grant-funded position ending March 31, 2027.
Position Summary :
We seek a Program Coordinator who will support our WYNYC SCN service delivery operations. These activities require close integration of workflows and effective relationship management with network partners across various sectors. The Program Coordinator will work closely with the Senior Director of WYNYC SCN, Deputy Director of WYNYC SCN and other members of the team to support the planning, execution, monitoring, evaluation and documentation of network service delivery activities.
Specifically, the Program Coordinator, WholeYouNYC SCN will assist with the planning and coordination of SCN service delivery programmatic and operational activities, including, but not limited to :
- Develop and monitor implementation of program-specific policies and procedures.
- Assist with the tracking, collection and analysis of network data to support evaluation activities.
- Develop and implement network monitoring processes and tools.
- Develop internal and external-facing network communications (e.g., newsletters).
- Assist with the coordination, scheduling and execution of network meetings and events.
- Create and maintain audience-specific network presentations and materials.
- Support programmatic budget management, resource allocation and fiscal reporting.
- Support the planning and execution of network quality assurance and improvement initiatives.
- Prepare and submit key programmatic requests including purchase orders, reimbursement requests, staff allocation, arrival / departure notices for management review and approval.
- Develop and manage a web-based SCN knowledge hub for network partners, organizing relevant supporting materials and documents for ease of access.
- Act as a liaison with other PHS operational and neighborhood health programs to support requests for information and enhance cross-organizational SCN coordination.
- Contribute to the timely and accurate completion of network deliverables, including work plans, reports, data collection and quality improvement activities.
- Under guidance from SCN management, plan and execute special projects that enhance network operations.
- Other duties as assigned by the Senior Director or Deputy Director to include support for CBO training and technical assistance, other logistical and programmatic support.
