PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.
New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.
This is a grant-funded position ending March 31, 2027.
Program Description
Family Connect is a specialized sub-network for pregnant and newly parenting New Yorkers that is part of the larger WholeYouNYC social care network. Family Connect receives referrals from health and social service providers for pregnant and newly parenting NYC residents, screens them for their needs and interests, and refers them to home visiting programs, doulas, and many other services and supports utilizing an electronic referral management system, Unite Us among other platforms. The goal of Family Connect is to connect families with services by connecting to community and clinical services and addressing health-related social needs to ultimately improve maternal and infant health outcomes.
Summary Of Position
Public Health Solutions is seeking a Program Manager to oversee the expansion of Family Connect’s screening and referral navigation to and effectively. The Program Manager will hold ongoing trainings for partners to build their capacity, conduct routine quality assurance to identify areas of improvement and support the facilitation of quality improvement learning collaboratives with PHS’ Healthcare Community Partnerships Capacity Building team. They will provide trainings and technical assistance to Family Connect partners accurately utilizing the referral management platform, Unite Us, and support the submission of billing claims for services rendered.

Specifically The Program Manager, Family Connect Capacity Building Will
- Lead and facilitate ongoing trainings and learning collaboratives for Family Connect community based partners to implement the Family Connect model;
- Conduct ongoing quality assurance and quality improvement to identify network issues, facilitate improvement cycles;
- Supervise one Family Connect Capacity Building Coordinator to support the expansion of the Family Connect screening and referral model;
- Provide technical assistance to Family Connect community-based partners;
- Develop and distribute protocols and guidance on Family Connect screening and referrals, utilizing the referral management platform and submitting billing claims for services rendered via Unite Us .
- Other tasks as needed.
- Master's degree or bachelor's degree with 3-5 years of experience;
- 1-2 years of experience conducting quality improvement and/or technical assistance ;
- Excellent organizational skills and the ability to coordinate multiple tasks, projects and assignments with competing priorities;
- Must have excellent written and oral communication skills, including comfort with public speaking and meeting facilitation;
- Demonstrated commitment to, and interest in learning more about maternal child health, systems approaches to health equity, reproductive justice, and community services in NYC;
- Previous experience with quality improvement preferred ; and
- Proficient in Microsoft Suite Applications (Word, Excel, SharePoint, etc.) and ability to learn and navigate new data systems
- Salary: $80,000 - $80,000
