Resource Specialist

Pay Range: $25 - $28/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs.
• The caseload is 100 members.
• This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.

Key Requirements and Technology Experience:

• Skills-Bachelor's degree,Child welfare background and Pediatric case management.
• Pediatric case management experience Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.
