Case Coordinator

Tapestry is seeking a Case Coordinator to join their amazing team in Brevard, NC!

Hourly Rate: $21.00/hour

Schedule: Monday-Friday 9am-4:30pm

• Tapestry Brevard will be moving to Fletcher NC in the next couple of months. Candidates will need to commit to driving to our Brevard location during the transition to our new facility in Fletcher NC.*
  
  

Essential Duties And Responsibilities

• Coordination of client engagement through appointments, aftercare plans, schedules and meetings in collaboration with treatment team.
• Coordination and communication with referral sources and outside providers through consistent, professional communication, assessment of needs, and documentation of contact at admission and for the
  
  

duration of treatment. This may include communication or coordination for UR, legal, and employment needs for the client.

• Providing individual case coordination sessions and services including coordination of transportation
• Assists in development of aftercare plan, providing referral and aftercare provider contact, addressing outside provider needs.
• Assists with groups to support clinical services provided, and supporting clients on a weekly basis to support engagement in the program.
• Documentation of client, family, referral, and outside provider contacts in Carelogic in accordance with program timelines
• Collaboration with client navigation team for client aftercare needs.
• Provides onsite support for Enhanced Outpatient Sessions (EOS) appointments between clients and Client Navigation team.
• Other duties as assigned
  
  

Education, Licensure, & Experience

• High School Diploma or equivalent required.
• 1 year in a drug and alcohol or mental health setting preferred.
• Driver’s License preferred
• CPR/BLS preferred.
