Key Job Duties include:
- Plan, develop, organize, implement, evaluate, and direct the social service and activity programs of the facility.
- Keep abreast of current federal and state regulations.
- Implement policies and procedures for the identification of medically related social and emotional needs of the resident.
- Participate in community planning related to the interests of the facility and the services and needs of the patient and family.
- Participate in discharge planning, development and implementation of social and activity care plans.
- Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting to the administrator as required.
- Interview residents/families as necessary and in a private setting. Involve family in planning when possible.
- Assist in arranging transportation to other facilities when necessary.
- Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.
Education
- Must possess, as a minimum, a High School Diploma or GED. A Bachelor's Degree in Social Work is preferred.
Experience
- Must be a qualified therapeutic recreation specialist or an activities professional who is licensed by the State of Arkansas and is eligible for certification as a recreation specialist or as an activities professional; or
- Must have, as a minimum, 2 years experience in a social or recreation program within the last 5 years, 1 of which was full-time in a patient actives program in a health care setting.
