Key Job Duties:
Coordinate Patient Assessments
- Conduct and document patient assessments (MDS) in accordance with regulations.
- Collaborate with interdisciplinary teams to develop individualized care plans.
- Ensure all assessments are completed within required timeframes and accurately reflect patient needs.
Lead Care Plan Development
- Chair the Interdisciplinary Care Plan Team to create holistic and patient-centered care plans.
- Update care plans regularly to reflect changes in patients’ conditions or preferences.
- Encourage patient and family involvement in the care planning process.
Ensure Compliance and Quality
- Implement quality assurance measures to maintain high standards in assessment and care planning.
- Participate in surveys and inspections, ensuring readiness and compliance with all regulations.
- Train staff on assessment protocols, care planning techniques, and related best practices.
Collaborate Across Teams
- Work closely with department heads to align care plans with nursing and medical goals.
- Facilitate communication between patients, families, and care providers to ensure cohesive care.
- Serve on facility committees, contributing to the development of policies and quality improvement initiatives.
Qualifications
- Active nursing license (RN or LPN) in the state of Arkansas.
- Experience in MDS coordination or care planning, preferably in long-term care.
- Knowledge of federal and state regulations related to patient assessments and care plans.
