Your Role and Responsibilities:
As a Registered Nurse with Quality Care Home Health & Hospice, you will play a crucial role in planning, organizing, and directing both home health services. You will:
- Conduct comprehensive assessments of patients and families to determine care needs
- Develop, implement, and regularly update personalized care plans, ensuring they address evolving patient needs.
- Administer medications, treatments, and provide health care instructions as prescribed.
- Assist patients with daily living activities, promoting self-sufficiency and comfort.
- Serve as a Case Manager, coordinating care across the healthcare team and ensuring seamless service delivery.
- Communicate effectively with physicians, interdisciplinary teams, and community health providers to coordinate care.
- Maintain accurate clinical notes and update relevant parties on patient status and care plans.
- Participate in on-call duties, ensuring patients have access to care when needed.
- Supervise and evaluate care provided by home health aides.
What We’re Looking For:
We’re seeking a Registered Nurse who is not only experienced but also passionate about home health. The ideal candidate will have:
- A degree from an accredited school of nursing,
- Current NH RN licensure and CPR certification.
- A Bachelor’s degree with at least one year of home care experience is preferred.
- Strong observation, communication, problem-solving, and nursing skills.
- Physical ability to walk, stand, lift, position, and transfer patients, as well as handle equipment and supplies.
- A valid driver’s license, a reliable and insured automobile, and a commitment to traveling as needed.
