Job Overview
We are seeking compassionate, dedicated individuals to join our team as Sponsored Residential Provider. This position offers the opportunity to make a significant, lasting impact in the life of an individual with intellectual and/or developmental disabilities by providing them with a safe, loving home environment and personalized care. As a Sponsored Residential Provider, you will serve as both a caregiver and a mentor, integrating the individual into your family while supporting them in achieving greater independence and quality of life.
**Key Responsibilities:**
- **Care for an Individual with Intellectual Disabilities:** Provide full-time care for an intellectually disabled individual within your home, promoting their physical, emotional, and social well-being.
- **Assistance with Daily Living Skills:** Assist the individual with personal hygiene, bathing, dressing, and other essential activities of daily living.
- **Medication Management:** Administer medications as prescribed, ensuring compliance with health protocols and support plans.
- **Medical Appointments:** Accompany the individual to doctor appointments and ensure their health needs are met.
- **Community Integration:** Facilitate community outings, events, and social activities to promote inclusion and participation in daily life.
- **Documentation:** Maintain accurate records of daily activities, progress, and health-related information, ensuring confidentiality and compliance with regulations.
- **Provide 24-Hour Care and Support:** Offer supervision and care depending on the individual’s needs, ensuring their safety and well-being at all times.
**Minimum Requirements:**
- High School Diploma or equivalent (GED).
- A minimum of 1-2 years of experience working with individuals with intellectual disabilities or in a related human services field.
- Must be 18 years or older.
- Valid Driver’s License and reliable transportation.
- Ability to work independently and complete necessary documentation.
- First Aid/CPR Certification and Medication Management Certification are a plus (training provided).
- Must pass background check and criminal history check.
- Willingness to integrate an individual into your home and family as a long-term care commitment.
- Compassion and passion for supporting individuals with disabilities.
**Essential Skills & Qualities:**
- Strong communication skills, both verbal and written, with the ability to maintain effective working relationships.
- Ability to promote self-determination and independence in individuals receiving care.
- Comfort with assisting with daily activities, behavior management, and providing emotional support.
- Knowledge of the DD Waiver, HCBS setting rules, and regulations preferred but not required (training available).
- Ability to model compassion, advocacy, and inclusivity in all interactions.
- Ability to maintain confidentiality and respect for the individual’s personal and health information.
