Clinical Manager/Quality Assurance
JOB SUMMARY:
The Clinical Manager/Quality Assurance is a Registered Nurse (RN) who has graduated from an accredited school of nursing and is currently licensed to practice in the state(s) where currently practicing. She/he supervises home health services to homebound patients in their place of residence in accordance with attending physician orders and plans of care and strives to provide the highest quality of care. RN works directly under the Director of Clinical Services. Clinical Manager/Quality assurance monitors all oasis/clinicians documentation.
QUALIFICATIONS:
1. Registered by the state(s) where currently practicing.
2. Two to five years of experience in health care/home care, preferred.
3. Two years' experience in a supervisory position, preferred.
4. Knowledge and ability to apply community health principles and practices.
5. Knowledge of Agency policies and procedures.
6. Ability to audit clinical documentation
7. Ability to assist with supervising, guiding and developing skills and performance of service personnel.
8. Ability to exercise independent judgment.
9. Ability to work with individuals.
10. Ability to enlist cooperation of many people in furthering a program.
11. Ability to assist in evaluating personnel per Agency policy.
RESPONSIBILITIES:
1. Audit Oasis/clinician documentation for compliance and accuracy
2. Participates in developing standards which ensure safe and therapeutically effective service to patients and families. Has responsibility to assist Clinical Director for seeing that standards are met.
3. Participates in developing objectives for Agency.
4. Monitor Quality Measure to look for areas for improvement
5. Assist the Clinical Director and IDT with QAPI program
6. Participates in planning for the orientation of new employees. Conducts selected orientation classes.

7. Displays a willingness to support the policies and procedures and uses appropriate channels for change of such policies; establishes criteria and procedures for selection, promotion and termination of employment of service personnel.
8. Assures ongoing assessment of patient/family needs and implementation of interdisciplinary team plan of care.
- Assures physician approval of plans for continuity of care.
9. Other administrative functions as assigned.
10. Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performance.
11. Observes confidentiality and safeguards all patient related information.
12. Accepts responsibility for regular attendance and punctuality and fulfills job requirements without regard to time involved.
13. Serve e as a resource person to employees.
14. Immediately reports any accident, incident, lost articles or unusual occurrence to the Clinical Director/Administrator.
15. Attends pertinent continuing education programs other than routine inservices and shares information with staff.
16. Assist with Monitoring equipment for appropriate use and take steps to keep misuse to a minimum.
17. Assist with Agency's ongoing Quality Assessment and Performance Improvement (QAPI) program.
18. Assist with overseeing development of indicators with appropriate data collection, aggregation and analysis, taking action and reporting results according to Agency's QAPI
19. Assures that patients' plans of care are developed, implemented and updated.
20. Assist with home visits as needed
WORk ENVIRONMENT:
Works indoors in the home health office. Occasional Home visits in patients home

Ability to perform the föllowing tasks if necessary:
Ability to participate in physical activity.
Ability to work for extended period of time while standing and being involved in physical activity.
