Job Summary
A Registered Nurse or Registered Therapist assists the Director of Clinical Services/Administrator with the supervision of home health services to patients in their place of residence in accordance with the physician orders and strives to provide the highest quality of care. S killed nursing and other therapeutic services are under the supervision of the Clinical Manager/Quality Assurance . The Clinical Manager /Quality Assurance is always available during operating hours (or appoints a similarly qualified alternate) and participates in all activities relevant to professional services furnished, including the development of qualifications and assignment of personnel. The Clinical Manager/Quality Assurance is a professional who focuses on the development and ongoing administration of the quality assurance and performance improvement (QAPI) program.
Qualifications
- Graduate of approved school of professional nursing or therapy school and currently licensed in the state(s) in which practicing.
- A qualified health care professional.
- Three to five years of experience in health care/home care, preferred.
- Acceptance of philosophy and goals of Agency.
- Ability to exercise initiative and independent judgment.
- Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal.
Responsibilities
- Understands and adheres to established agency policies and procedures.
- Understands and promotes principles of continuous performance improvement.
- Participates in the development of standards, which ensure safe and therapeutical effective service to patient/family.
- Ability to supervise, guide and develop nursing skills and performances.
- Consults with the D irector of C linical S ervices/ A dministrator and determining a staffing pattern, which will accomplish stated adjectives and promote a maximum level of utilization of health personnel.
- Assist in recruiting, hiring, evaluating and terminating nursing personnel. Assists in instituting, interpreting, and documenting disciplinary action.
- Responsible for orientation of new patient care personnel.
- Responsible for the orientation of new staff to agencies QAPI program.
- Assist in planning and consultative needs of staff.
- Assist in the preparation and implementation of policies and procedures, which mean Medicare, Medicaid, accrediting bodies, sta te and local laws.
- Displays a willingness to support policies and procedures and uses appropriate channels for changes of such policies.
- Assist in the coordination of interdisciplinary team services, patient care, and referrals.
- Assist in the organization of nursing services to delineate and delegate functional responsibility, lines of relationship and communication to provide safe and therapeutical effective service.
- Participates in the coordination of Agency’s services with the services of other community agencies.
- Participates in studies and research and other administrative functions as assigned.
- Serves as a role model for all colleagues by setting an example of high standards of dress, conduct, cooperation, and job performance.
- Observes confidentiality and safeguards all patient related information.
- Accepts responsibility for regular attendance and punctuality fulfills job related requirements without regard to time involved.
- Serves as a resource person to employees.
- Develops a cooperative relationship and communicates effectively and professionally with physicians.
- Investigate and reports any problem relating to patient care and or employee well-being.
- Immediately reports any accident, incident, lost articles, or unusual occurrence to the D irector of C linical S ervices/ A dministrator.
- Attends pertinent continuing education programs and other than routine inservices and shares information with staff.
- Monitors equipment for appropriate use and take steps to keep misuse to a minimum.
- Assist in the development of process improvement activities with appropriate data collection, aggregation, analysis, taking action and reporting of results according to Agency’s QAPI plan.
- Assures the development, implementation, and updates of the individualized patient plans of care.
- Reviews, patient medical records for compliance with federal, state, accrediting, bodies, local and agency policies and guidelines.
- Assist D irector of C linical S ervices in conducting patient care conferences, staff meetings and maintains documentation. Participates in community education projects.
- Ensure that all necessary supplies and equipment are available.
- Must be able to perform nursing functions in times of staff shortage.
- Available to share on - call with nursing personnel as needed.
- Ensure that documentation is completed in accordance with A gency policy.
- Make staff and patient assignments and on-c all.
- Chairs QAPI Committee and prepares QAPI reports/minutes of meeting and forwards reports according to QAPI plan.
- Other duties as assigned by the D irector of C linical S ervices/ A dministrator.
- Act as D irector of C linical S ervices in the D irector’s absence.
- A ssures that the patient needs are continually assessed.
Working Environment
Work indoors in the home health office.

Workers Supervised: Entire home health patient care staff /QAPI Committee

Ability to perform the following tasks if necessary:
- Ability to participate in physical activity.
- Ability to work for extended periods of time while standing and being involved in physical activity.
- Moderate lifting.
- Ability to do extensive bending, lifting and standing on a regular basis.
Salary : $70,000 - $72,000
