Homeless Housing Case Manager Immediate Starts!
- Homeless Housing Case Manager will report to the Supervisor.
- Housing Case manager responsible for engaging individuals experiencing homelessness and supporting them to ensure that they are successfully connected to a permanent housing opportunity.
- The housing case manager would be responsible for assessing the client for the transition to work-helping to move clients out of long-term care facilities and into their own apartments.
- The housing case manager work with housing specialists.
- The housing case manager would show the apartments to client so they can make a choice.
- The housing case manager will coordinate the move, help pick out furniture and supplies, get utilities hooked up etc.
- They would provide linkage/referral to needed services once the client was in the community and address any other needs they have.
- Great Organization with room for growth
Must have a Masters Degree in a human service field with 1 year in the social service field.
Must have a valid Drivers Licenses and reliable transportation
Salary : $26
