Primary Purpose
Serve as a community-based healthcare support person delivering paraprofessional primary care support, disease prevention services, and health education, including culturally relevant perinatal support. This role integrates traditional Native concepts with outreach, advocacy, and support services for individuals and families in the Quartz Valley area under the direction of the Registered Nurse.
Essential Duties and Responsibilities
General Community Health Responsibilities
- Community Outreach and Communication: Facilitate communication between community members and healthcare providers, acting as an advocate to improve access to care.
- Health Assessments and Screenings: Perform basic assessments such as blood sugar levels, vital signs, vision screenings, and other screenings as needed.
- Health Education: Provide education on diabetes, hypertension, immunizations, and health promotion, in individual or group settings.
Perinatal-Specific Responsibilities
- Perinatal Education and Support: Offer prenatal care education, nutrition information, breastfeeding support, family planning resources, and parenting resources.
- Emotional and Mental Health Support: Provide access to mental health resources such as prenatal and postpartum anxiety or depression. Provide emotional support for clients facing barriers or crises, by connecting them to appropriate care professionals.
- Resource Navigation: Assist clients in accessing perinatal healthcare, housing, food assistance, childcare, transportation, and other community resources.
- Cultural Competency in Care: Provide culturally sensitive services, integrating traditional practices and beliefs into health education and care delivery.
Administrative Duties
- Case Management and Follow-up: Track client progress during pregnancy and post-natal periods, ensuring adherence to healthcare plans.
- Data Collection and Reporting: Maintain accurate records in EHR/RPMS systems and report on health outcomes and program participation.
- Grant Management: Support and meeting required deadlines and deliverables.
Program and Community Integration
Collaboration: Work with healthcare providers, social workers, tribal councils, and community organizations to enhance care coordination.
- Program Development and Oversight: Participate in regular reviews to assess program impact and effectiveness.
- Training and Development: Engage in ongoing education on perinatal care trends, cultural competency, and community health practices.
Home Evaluations for patient quality of living.
Other duties as assigned.
Supervisory Responsibilities
This position has no supervisory responsibilities.
Education and Experience Requirements
Education:
· High school diploma (or equivalent). Required
Proficiencies:
· CPR and First Aid certification within 90 days of employment.
· Completion of Basic and Advanced HIS we-based training modules within 6 months.
· Read, understand, and apply written policy, procedures and directions.
· Ability to effectively use a variety of computer programs needed to perform duties.
· Adherence to confidentiality and HIPAA policies and procedures.
· Must pass Medical department competencies withing 6 months of hire.
Experience:
· 1 year experience in related field.
Other Requirements
· Must be able to pass background check.
· Must adhere to Tribal drug and alcohol policy, including pre-employment drug and alcohol screening.
· Must have a valid driver's license and insurable under the Tribe's insurance policy.
