We are seeking a compassionate Social Work Care Specialist to join our Care Management team. In this position, you will be responsible for assisting members to achieve optimal health and/or independence in managing their care and develop various care plans for self-care which will including motivational assessment, assessing for desired level of involvement, and coaching for adherence to care plan.

- Starting salary based upon skills and experience: $65,300 - $81,700 plus robust benefits package.
Responsibilities
- Conducting outreach to eligible members who need extra support, helping them understand and access available healthcare services.
- Assess for social determinants of health, including but not limited to, social/family/caregiver support, housing needs, financial needs, and behavioral health needs.
- Establish relationships and advocate for member services and needs with primary care providers and comprehensive care teams, including promotion of culturally appropriate care and services, and education on targeted P4P measures.
- Measure progress, reassess, and modify care plans and goals with clients to achieve desired health outcomes and established goals.
- Ensure precise, comprehensive, and timely documentation in appropriate systems following established workflows and processes.
- Participate in determining and promoting annual performance improvement projects (PIPs).
- Provide expert knowledge of community and/or agency resources to assist individuals with achieving the highest possible level of independence, functioning, and/or self-care management of their condition(s).
Qualifications
- 2 years of formal experience working with population that including intellectually/developmentally disabled, physically disabled, and/or the frail elderly.
- Bachelor’s degree in social work (BSW) or a related human services field is required.
- 3 years of complex / chronic condition case management experience working in health and/or social services in a healthcare setting.
- Experience with Electronic Medical Records, with the ability to work independently in Microsoft Office applications.
- Experience with Motivational Interviewing and Trauma informed care.
- Experience with Epic and/or Altruista.
- Strong working knowledge of healthcare measures and initiatives set by DHS for the Medicaid population throughout the service area(s).
- Knowledge of and ability to work within a complex health care system including advocating for member needs while balancing organization needs.
- Knowledge of local, state, and federal community resources available to members, with the ability to assist members/families access the resources.
- Social Worker Certification (CSW), Valid Driver’s License is required.
