Job Summary
We are seeking a dedicated and compassionate Specialist to join our team. Responsibilities
- Provide direct support to individuals with disabilities, ensuring their needs are met in a respectful and empowering manner.
- Assist in the development and implementation of individualized support plans that promote independence and personal growth.
- Collect and analyze data related to client progress, behaviors, and overall well-being.
- Communicate effectively with clients, families, and interdisciplinary teams to ensure coordinated care.
- Facilitate activities that promote social interaction, skill development, and community involvement.
- Utilize American Sign Language as needed to enhance communication with clients who are deaf or hard of hearing.
- Support individuals on the autism spectrum by employing tailored strategies that foster engagement and understanding.
- Collaborate with educational institutions to provide support within special education settings as required.
- Maintain accurate records of client interactions and progress in compliance with organizational policies.
- An IDD Case Specialist, or Case Manager for Intellectual and Developmental Disabilities, is a professional who assists individuals with IDD in accessing and coordinating various services and supports to improve their quality of life and community participation. They work with individuals and their families to identify needs, develop individualized plans of care, and ensure the delivery of appropriate services.
- Assessments:Conduct assessments to identify the individual's needs, strengths, and preferences.
Services Coordinated by IDD Case Specialists:
- Medical Care: Access to healthcare professionals and services.
- Educational Services: Access to special education programs and support.
- Therapy: Access to therapies such as speech therapy, occupational therapy, and physical therapy.
- Day Programs: Access to day programs that provide socialization, skill-building, and recreational activities.
- Housing: Access to appropriate housing options, including residential care facilities and supportive housing.
- Vocational Services: Access to vocational training and job placement services.
- Transportation: Access to transportation services to attend appointments and activities.
- Individualized Plans of Care: Develop and update individualized plans of care based on the individual's needs and choices.
- Service Coordination: Coordinate and monitor services from various sources, including medical, social, educational, and other support programs.
- Advocacy: Advocate for the individual's rights and needs, ensuring access to appropriate services and support.
