Responsibilities
The Quileute Tribe is seeking a skilled and compassionate Community Health Representative to join our Quileute Health Center team.
POSITION SUMMARY:
The Community Health Representative (CHR) serves as a key liaison between the tribal community and the Quileute Health Center, focusing on improving health outcomes through education, advocacy, and direct support. The CHR will work to enhance community health by delivering culturally relevant health education, facilitating access to healthcare services, and supporting the overall well-being of community members. This role involves a combination of outreach, support, and coordination activities tailored to the unique needs of the tribal community.
ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:
- Engage with community members to provide information about health programs, services, and resources available at the Tribal Health Center. Conduct outreach activities to increase awareness and participation in health initiatives.
- Provide culturally appropriate health education on topics such as disease prevention, wellness, nutrition, and chronic disease management. Develop and deliver educational materials and presentations that address community-specific health concerns.
- Assist community members in navigating the healthcare system, including scheduling appointments, understanding medical instructions, and accessing necessary resources. Offer support for managing chronic conditions and adhering to treatment plans.
- Advocate for community members’ needs within the healthcare system. Provide case management support to help individuals access services and overcome barriers to care, including transportation, financial assistance, and language interpretation.
- Collect and maintain data related to community health needs, program participation, and outcomes. Prepare reports and provide feedback to help improve health services and programs.
- Ensure that all interactions and services are respectful of and aligned with tribal customs, traditions, and cultural practices. Foster trust and build strong relationships with community members through culturally sensitive approaches.
- Collaborate with healthcare providers, community organizations, and other stakeholders to coordinate and enhance health programs and services. Participate in community health events and activities to promote engagement and awareness.
- Connect community members with additional resources and services as needed, including social services, mental health support, and preventive care programs.
- Upholds, implements, and adheres to Quileute Tribal Policies, Laws, and Regulations as approved by the Quileute Tribal Council.
- Provide patient transports to medical and dental appointment if needed, this could include weekend or overnights.
- Maintenance of BLS Certification
- Other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED FOR THE POSITION:
- Deep understanding and respect for tribal traditions, customs, and cultural practices. Ability to provide services that are culturally relevant and sensitive.
- Strong verbal and written communication skills, with the ability to effectively engage with diverse community members and collaborate with healthcare professionals.
- Excellent organizational and time management skills, with the ability to manage multiple tasks and priorities efficiently.
- Ability to build rapport, demonstrate empathy, and support community members in a respectful and non-judgmental manner.
- Basic understanding of health and wellness concepts, community health issues, and available resources. Familiarity with local health services and resources is a plus.
· Must maintain strict confidentiality of medical information and adhere to HIPAA and Privacy Act requirements.
- Demonstrates respect for and awareness of Quileute customs, traditions and historical background.
- Strong computer skills, proficiency in Microsoft Excel, Outlook and Word.
ENVIRONMENT:
· Work involves interacting with community members in various settings, including the Tribal Health Center, community locations, and home visits.

REQUIRED QUALIFICATIONS:
· High School Diploma or GED Equivalent
- Experience in health clinic or healthcare setting
- Current BLS or obtain with 6 months of hire
