Job Summary
Racker is seeking a skilled Community Support Specialist to provide community-based services for individuals with developmental disabilities and their families. The ideal candidate will work in accordance with service plans and individual interests, collaborating with the service recipient and their family to ensure the highest level of support.
Key Responsibilities
- Provide individualized services to people with disabilities and their families, including respite, teaching, modeling, and skill development in various areas.
- Participate as an active member of the person's team, offering ideas, input, and advocacy related to their Staff Action Plan.
- Attend training to improve services and complete job requirements within designated time frames.
- Report any potential incidents to the supervising Fiscal Intermediary or Self-Direction Coordinator.
- Accurately enter time and services into Rackers electronic health record, eVero, documenting service times, supports provided, and mileage according to program and regulatory requirements.
- Communicate regularly with the person being supported and their team regarding service provision.
- Uphold standards of professionalism with regard to behavior and dress.
Working Relationships
- Responsible to the person receiving services, their identified designee/family, and the assigned Fiscal Intermediary.
- Member of the team, which may include the person and their family members, circle of support, Care Manager, Support Broker, Fiscal Intermediary, teachers, therapists, and other providers as selected by the person receiving services.
Qualifications
- High school diploma or GED.
- Meet Approved Driver Status.
