Salary Range $64,000 - $89,000 Annual
Position Summary: Under direction of the Clinic Director or his/her designee, The Population Health Nurse is a professional RN whose role is to coordinate care for patients within a defined population. The Nurse facilitates the attainment and maintenance of an optimal level of health for these patients by collaborating
with other health team members and the community resources to ensure the continuity of care for individuals and their families across the continuum. The Population Health Nurse requires a high level of customer service skills to establish and enhance positive relationships with patients, co-workers and others. This position will also manage the clinic Coumadin patients, do the diabetes education for the facility, assist the clinic nurses in triaging patients and patient care coordination when needed and collaborate with patient education needs as it corresponds to the care coordination.
