Blended Hourly W2 Pay Rate: $52.00
Blended Weekly Gross Pay Rate: $2,080.00
Minimum Hourly W2 Pay Rate: $17.70
Min. W2 Weekly Gross (Taxable): $708.00
Weekly Stipend (Non-Taxable): $1,372.00

Pay Rate on 1099 to the Candidate: $54.00
Weekly 1099 Traveler Pay Rate: $2,160.00
Overview:
We are seeking a compassionate and skilled Social Worker to join our healthcare team and support patients and families through their healthcare journey. This position plays a vital role in providing psychosocial services, discharge planning, crisis intervention, and advocacy in a fast-paced hospital environment.
Key Responsibilities:
- Deliver social services to patients and families within an assigned medical service area.
- Conduct discharge planning evaluations and collaborate with hospital staff to develop effective care transitions.
- Perform psychosocial assessments and share findings with interdisciplinary teams.
- Provide emotional and practical counseling related to diagnoses, treatment plans, financial concerns, and family issues.
- Identify high-risk social cases, such as abuse or neglect, and offer crisis counseling while thoroughly documenting interactions.
- Act as a liaison between patients/families, hospital personnel, and external agencies or community resources.
- Evaluate the effectiveness of services provided and adjust care plans to optimize patient outcomes.
- Support patients and families during transitions to post-acute care or home-based services.
- Address complex emotional reactions to illness, hospitalization, and loss of independence.
- Utilize specialized communication skills, particularly when working with pediatric patients.
- Serve as a patient advocate, particularly in areas related to informed decision-making, end-of-life care, and ethical concerns.
- Maintain accurate records and contribute to reports related to referrals, extended stays, and care coordination.
- May supervise social work interns or newly hired staff members.
Qualifications:
- Education: Master’s Degree in Social Work (MSW) from an accredited institution.
- Licensure/Certification: LMSW strongly preferred.
- Experience: Prior experience in a hospital or healthcare setting is preferred.
- Must have experience with inpatient hospital discharge planning, including coordinating placements in LTACs/SNFs, arranging durable medical equipment (DME), setting up home health services, and collaborating with agencies such as DHS and APS.
Required Skills & Abilities:
- In-depth knowledge of social work and case management principles.
- Familiarity with community and hospital-based social and welfare resources.
- Ability to assess, problem-solve, and implement interventions in a clinical setting.
- Strong communication skills with the ability to explain complex information to patients, families, and staff.
- Excellent interpersonal and collaboration skills.
- Able to handle sensitive and emotional situations with professionalism and empathy.
