What are the responsibilities and job description for the Activities Coordinator - Swing Bed position at Rangely District Hospital?
JOB STATEMENT:
Provides a quality activity program for the Long-Term Care Residents.
JOB DUTIES:
Prepares the monthly calendar of events that is to be posted in the resident hall.
Prepares an activity plan for each resident on admission and re-evaluate quarterly. Each plan must be co-signed by the DON. The plan must be individualized to meet the needs, problems, and limitations of each resident; include 1:1 and group activities; set goals and document how well these goals were met.
Maintains documentation for daily activities. Other staff members will also document activities or leave information for the activity coordinator to document.
The activity coordinator will oversee the activity program, discuss problems with the DON or Assistant to CNO/DON.
Performs other related duties as required, requested, or assigned.
JOB SPECIFICATIONS:
High School Graduate or equivalent.
CPR Certification.
National Association of Activity Professionals (NAAP) Certification, or willingness to obtain certification within 60 days of hire.
CNA Certification is required within one year if not already certified.
Prolonged, extensive, or considerable standing/walking, lifts, positions, pushes and/or transfers patients, lifts supplies/equipment, considerable reaching, stooping, bending, kneeling or crouching.
Contact with patients under wide variety of circumstances. Will be exposed to patient elements. Subject to varying and unpredictable situations.
