OVERVIEWProvides direct care in the agency's supported living and sponsored placement program for individuals with intellectual and developmental disabilities. Work is performed under regular supervision and is guided by well-established and specific agency policies and procedures, quality assurance, public and private payor guidelines, professional practices and including ethical standards related to client interactions, state and federal licensure and accreditation regulations and guidelines.DUTIES AND RESPONSIBILITIESDevelops and monitors individual service plans in collaboration with providers, families, and other service providers, as appropriate.Instructs and trains providers in individual service plan facilitation and required documentation. Monitors and evaluates individual's progress and prepare reports for other team members. Makes recommendations regarding program goals and issues and links individuals to services external to the agency. Provides crisis intervention services.Completes case notes in the Electronic Health Record.Reviews, verifies / corrects, and scans provider produced documentation into Electronic Health Record. Alerts the Sponsored Placement Supervisor to any missing documentation.Monitors progress of provider training requirements.Creates and manages individual's budgets, writes and distributes checks, manages requisitions for projected funds needed and prepares checkbooks for monthly audit,Completes paperwork pertaining to the individual's benefits or other services received.Provides medication management support to providers and monitors all medication changes and concerns.Calculates and enters Medicaid billing and provider payments and collects payments.Conducts sponsored placement home visits and reviews provider documentation for licensure and regulation compliance. Attends / completes required training. Does related work as assigned.KNOWLEDGE, SKILLS, AND ABILITIESThorough knowledge of behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning and service coordination relevant to the intellectually-disabled population; thorough knowledge of individuals' rights; thorough knowledge of local community resources and services available to the intellectually-disabled population; considerable knowledge of social casework techniques, service planning process and related documentation; considerable knowledge of interviewing and supportive counseling techniques; considerable knowledge of the methods and use of electronic health records; considerable knowledge of state licensure requirements; ability to establish effective working relationships with a variety of others; ability to communicate effectively orally and in writing; ability to operate a vehicle; ability to conduct effective interviews, observe, assess and record individual behaviors and functioning and develop treatment and service plans; ability identify and coordinate client services; ability to remain calm in stressful situations. TYPICAL EDUCATION, TRAINING AND WORK EXPERIENCEBachelor's Degree in the Behavioral Sciences / Human Services and over 1 year work experience involving residential care services to people with intellectual or developmental disabilities is preferred. See Medicaid Billing Addendum.ENVIRONMENTAL AND PHYSICAL EXPOSUREWork involves some exposure to disagreeable environmental conditions such as the requirements to interact with clients having unsanitary behaviors, poor hygiene, or the potential for hostile and aggressive behavior; frequently driving a vehicle long-distances; regularly physically assisting and / or lifting and occasionally restraining clients in the course of care; is occasionally interrupted during off-hours with emergent calls requiring crisis intervention.RELATIONSHIPSRelationships require excellent communication and interpersonal skills, tact and the use of specialized techniques to collaborate with agency staff, coordinate services and elicit information from and convey information to program clients and their sponsoring providers.
