POSITION SUMMARY:
The Day Habilitation Facilitator is responsible for ensuring the members are active in the Day Habilitation and out in the community. This position helps train and supervise Direct Care Staff on individual member needs, and on some occasions works directly with the members. This position is responsible for much of the member paperwork such as the daily documentation, quarterly reports, service plans and file reviews. This position requires working in conjunction with the Day Habilitation Service Coordinator as well as the Member Health Care Coordinator, and direct care staff in order to ensure the highest quality of services.
ESSENTIAL JOB FUNCTIONS:
- Meet with members and develop a list of requested community and in-house activities.
- Maintain documentation that the member is choosing the activities that they would like to participate in while in the community and at the Day Habilitation.
- Develop monthly schedule of the community and in-house activities based on % of program time for each member.
- Distribute the schedule of community and in-house activities to the guardians monthly.
- Evaluate satisfaction for the activity and % of program time in the community for each member.
- Assist in planning services to meet individual needs of members including staff schedules and review of timesheets for accuracy.
- Attend Individual Service Plan review meetings as needed in the absence of the Day Habilitation/Adult Day Program Coordinator.
- Write member quarterly progress reports.
- Assist with maintaining member records, blue binders, including attendance records, member documentation, and goal checklists as needed.
- Assist with daily activities including toileting, feeding, hygiene needs, and recreational activities. Transport members as needed.
- Acquaint and orient new members to the day program.
- Provide ongoing staff orientation and training including goals, routines, and medical needs.
- Provide the Service Coordinator input on Direct Care staff performance as needed.
- Treat co-staff and supervisors with dignity and respect by listening to what others have to say and receiving constructive criticism.
- Drives team-related work and respects the ideas and opinions of team members. Presents themselves in a professional manner.
- Be on time and work scheduled days.
- Complete other duties as assigned.
JOB REQUIREMENTS:
EDUCATION/ EXPERIENCE:
High school degree (or its equivalent) and experience in working with adults with disabilities.
SKILLS:
Must be able to communicate effectively with members and staff. Patience in working with the members in necessary. Needs to have a caring attitude and be supportive and encouraging to members. A good driving record is necessary.
COVID-19 PRECAUTIONS:
All RFYP staff are required to follow the current masking procedures and use all required PPE as defined by RFYP Administration. All staff need to follow disinfecting procedures and hand washing policies. Staff need to be aware of how COVID-19 spreads, risk factors, and prevention actions. If staff show any signs of COVID-19 symptoms, they are to contact their supervisor or HR immediately.

EQUIPMENT TO BE USED:
Must be able to operate a computer and other office machines such as fax, calculator, telephone, and so on. Training for specialized adaptive equipment for individual members will be provided.
TYPICAL PHYSICAL DEMANDS:
It is preferred that the employee is able to lift at least 50 lbs. Must be willing and able to assist with toileting and other activities of daily living as required.
WORKING CONDITIONS:
Typically works in both an office setting and direct care in the day program.
