POSITION:       Wraparound Care Service Facilitator Trainee


LOCATION:      Mishomis Wellness Center  


SUPERVISOR: Lead Service Facilitator

                          Health Services Administrator


SALARY:           $18- $20 per hour 40 hours/week plus benefits 


THIS IS A REGULAR FULL-TIME NON-EXEMPT POSITION


JOB SUMMARY:  The role of this service facilitator trainee involves receiving training under the direct guidance of the service facilitator lead to develop proficiency in coordinating all elements of the assessment, planning, and service delivery process within integrated systems of care.  This ensures that services are effectively monitored, and outcomes are achieved for the individuals served.  The position is structured to assist clients in reaching their highest level of independent functioning.  In this position you will learn to apply holistic Wellness services that encompass physical, social, emotional, mental, and spiritual well-being. The services and care you provide will be guided by Wraparound Care principles, the Tiwahe Framework, and the 7 grandfather teachings.  You will learn to integrate a trauma informed and culturally integrated approach. Additional responsibilities will include learning to navigate systems of care, coordinating resources, leading teams, and facilitating services across various care systems to meet the overarching goals and objectives and outcomes of those supported.  

DUTIES AND RESPONSIBILITIES:

  1. Training and Development: 
  • Participating in training programs under the supervision and/or direction of the service facilitator lead to gain proficiency in service facilitation.
  • Engage in continuous learning about best practices in holistic service coordination and delivery.
  • Willingness to expand your educational opportunities and experiences by having the desire and ability to complete additional training in service facilitation concepts such as wraparound model, ethics and boundaries, motivational interviewing, etc.
  1. Assessment and Planning: 
  • Assist in and conduct assessments to identify the needs and opportunities to develop client (relative) centered, trauma informed, and culturally integrated service plans. 
  • Collaborate with the service facilitator lead to ensure integration of service and care across multiple systems.
  1. Service Delivery Coordination:   
  • Coordinate all aspects of service delivery ensuring that services are effectively monitored, and outcomes are met
  • Facilitate communication and collaboration among team members involved in the care of clients (relatives).
  1.  Holistic Wellness Services: 
  • Learn to apply holistic Wellness services that address physical, social, emotional, mental, and spiritual well-being.  
  • Ensure that the care provided aligns with Wraparound Care Principles, the Tiwahe Framework, and the 7 Grandfather Teachings.    
  1. Trauma Informed and Culturally Integrated Care: 
  • Implement a trauma informed approach in all interactions and service delivery.
  • Respect and incorporate cultural values and practices into the care provided to clients (relatives). 
  1. Resource Coordination and Navigation:
  • Gain knowledge in navigating systems of care and coordinating resources effectively for clients (relatives). 
  • Assist clients (relatives) in assessing available services, support, and opportunities.  
  1. Team Leadership:
  • Lead teams as needed to facilitate discussions and ensure that opportunities for care are identified and integrated into the care planning process. Ensure tasks are delegated and monitored to ensure achievement of targeted outcomes. 
  • Participate and eventually lead regular reviews of service plans and adjust as necessary to meet client (relative) needs.  
  1. Monitoring and Evaluation:
  • Assist and independently monitor client progress and evaluation of the effectiveness of the services provided.  
  • Participate in regular reviews of service plans and adjust as necessary to meet the needs and outcomes of those you serve. 
  1. Documentation and Reporting:
  • Maintain accurate and up-to-date records of client (relative) and collaborative interactions.  
  • Prepare reports as required to communicate client (relative) progress and outcomes to stakeholders.
  1. Community Engagement:
  • Engage with community resources and organizations to enhance service delivery and support client (relative) needs.
  • Promote awareness of available services and foster partnerships within the community. 
  1. Confidentiality: 
  • Maintain a confidential client record keeping system in accordance with program and state regulations and guidelines.
  1. Other Duties as Assigned: 
  • The above duties and responsibilities are not an all-inclusive list but rather a general representation of the duties and responsibilities associated with this position.  The duties and responsibilities will be subject to change based on organizational needs and/or deemed necessary by the supervisor.

SUPERVISORY AUTHORITY: None

KNOWLEDGE: 

  1. Good interpersonal skills and the ability to get along with diverse populations (clients, co-workers, professional staff, administration, and the public).
  2. Good communication skills, written and oral.
  3. Ability to document and track face to face contacts, phone calls, and other correspondence.
  4. Sound judgment and the capacity to respond to unusual circumstances.
  5. The ability to plan, coordinate, and direct varied and complex organizations.
  6. Awareness of and sensitivity to Ojibwe culture and traditions.


QUALIFICATIONS:

  1. Associate’s degree preferred but a minimum of High school diploma, HSED, or GED required. 
  2. Minimum of three years of experience working with programs servicing individuals, families, and or community required.  
  3. Minimum of 1 year of case management and/or care coordination services preferred. 
  4. Language and cultural lived or professional experience with tribal people required. 
  5. Must have a valid driver’s license, vehicle and appropriate vehicle coverage and maintain driver’s eligibility as a condition of employment.  Be eligible to be put on the tribe’s vehicle insurance policy. 

PERSONAL CONTACTS: Considerable daily contact will be made with the patients, general public, tribal leadership, Program Directors and staff, consultant(s), and the Tribal Community.


PHYSICAL REQUIREMENTS: The duties assigned to this position involve bending, stooping, lifting, and carrying. Items may be placed in overhead storage or shelving.


WORK ENVIRONMENT: All Tribal Buildings are smoke free.  Able to travel throughout the community, make home visits, attend meetings, and participate in community educational programs.  Private residences, when utilized for professional activities, will not necessarily meet the criteria of the established environmental safety regulations.


TRAVEL REQUIREMENTS: The ability to travel and attend meetings and trainings; overnight and out of town, both locally & nationally.
