What are the responsibilities and job description for the Intake Coordinator I position at RED ROCK BEHAVIORAL HEALTH SERVICES?
SUMMARY
The Intake Coordinator is part of a multidisciplinary clinical team, conducting phone and face-to-face screenings and intakes on-site, off-site, and via telehealth for veterans, guardsman, active-duty, and their families. The Intake Coordinator will work with the team for case assignments, consultations, and care coordination.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
- Conducts initial intake assessments with clients.
- Provides support to clients with completing initial consent forms and releases as required
- Provides warm hand-offs for clients needing external referrals
- Coordinates case assignments with multi-disciplinary team
- Thoroughly and efficiently completes documentation within required timeline of 48 hours of intervention/communication
- Collaborates with clinical team and participates in multidisciplinary team meetings
- Provides crisis management support to clients and the team as needed
- Provides services in-person and via telehealth platform, as required
- Perform other duties as assigned
QUALIFICATIONS

Master’s degree in social work, Sociology, Psychology or related field is required; Oklahoma Licensure or Candidate for Licensure for one of the following is preferred:
- Licensed Professional Counselor (LPC)
- Licensed Marriage and Family Therapist (LMFT)
- Licensed Clinical Social Worker (LCSW)
- Licensed Alcohol and Drug Counselor (LADC/MH)
- At least one year of experience providing intake coordination and/or case management services; experience working with a military or veteran population is preferred
- Knowledge of military connected and veterans service organizations and resources
- Ability to think analytically and evaluate the impact of case assignments
- Excellent written and oral communication skills
- Highly organized, detail oriented, and reliable
- Works effectively in and contributes to a healthy team environment
- Proficient in use of technology to perform duties
- Displays appropriate understanding of and adherence to policies and procedures and organizational hierarchy
- Ability to set priorities and evaluate and create solutions to work related problems
- Must have a valid driver’s licenses and reliable transportation
