Summary of Job
Assists in meeting the psychosocial needs of patients/families and enables patients to utilize health care in order to achieve their optimal level of health.

Recruitment Requirements
- Masters degree from an accredited school of social work.
- Eligible for licensure in New Jersey and/or PA.
- 1-2 years post graduate degree experience in a healthcare setting (hospital/home care preferred).
- Computer skills/knowledge required.
- Flexible sound judgment customer oriented organized keeps professional demeanor at all times.
- Excellent verbal and written communication skills.
- Available to work flexible hours with weekends and holidays included.
- Knowledge of general disease process.
- Knowledge of family dynamics and interpersonal relationships.
- Member of the Academy of Certified Social Workers signifying two years of practice under qualified supervision preferred.
- Able to travel to patients home within service area.
- Able to use office equipment i.e. beeper car phone.
- Valid driver's license.
