What are the responsibilities and job description for the Housing Case Manager position at Redeemer Services, Inc?
This role's primarily focus is to ensure that individuals (client) who we work can find and maintain long-term stable housing. You will be responsible for creating a housing plan based on the individual wants and needs.
You will work with the individual to develop a budget based on the individual's income; remove any barriers that might limit housing choices such as prior evictions, limited income, etc. Search for housing based on the identified needs on the housing plan. Take individuals to view potential options and meet landlords. Coordinate the move from the old to new home. Once the move is completed work with the individual to ensure that they maintain their new home.
Primary Job Duties
- Meet the individual and develop a housing plan based on the client's wants and needs.
- Remove any barrier that limit housing choices such as expunging eviction notices or income based limits by applying for other external funding sources such as Social security or any grants available through state and other charitable organizations.
- You will search for housing using available resources, identify possible choices based on the housing plan.
- Take the individual on tours to view identified housing options.
- You will usually be working with the individual's case manager to provide updates about the status of the case.
- Document and case note all activities daily.
- Fill application on behalf of the individual.
- Request necessary document as needed on behalf of the individual.
- Perform check-ins with the client as needed. Setup and coordinate any move once placement has been found.
- Provide updates to the individual's case manager.
- Communicate with the landlord on behalf of the individual as needed.
- Ensure that Release of information has been properly completed.
- Follow up with case managers regarding support plan.
Education & Experience:
- Associate degree or higher.
- Alternatively, a minimum of 2 years of college.
- Excellent verbal and written communications in English.
- Basic knowledge of Windows and iOS.
Requirements:
- Able to pass a Department of Human Services (DHS) Background Study.
- Valid Driver's License with less than 3 moving violations in the last 3 years, including DUI.
- Must be comfortable having clients in your vehicle.
- Maintain a reliable vehicle.
- Maintain valid car insurance.
- Able to work with diverse groups, including racial and disabilities.
Preferred Requirements:
- Knowledge of the local housing market.
- Experience working with people with disabilities and chronic illnesses.
- Critical thinking and problem-solving skills.

Schedule:
- 8-hour shift.
- Hybrid: Staff can gain up to 4 days working from based on meeting performance criteria.
- Day shift (8:30am - 5:00pm)
- Monday to Friday.
