Organization Description: Refugee & Immigrant Self-Empowerment, Inc. (RISE), formerly known as the Somali Bantu Community Association (SBCA), is a human service nonprofit founded by Somali Bantu refugees in 2004 to ease the transition into post-resettlement (90 days in-country) members of the Community throughout Syracuse. As its clientele grew increasingly diverse, RISE extended its post-resettlement services to all refugees and immigrants throughout Onondaga County. The majority of RISE is operated and governed by former refugees, delivering a variety of services which endeavor to promote self-sufficiency and autonomy through employment, education, and economic empowerment.
Job Summary
RISE Buffalo is seeking a field-based Care Manager to provide culturally and linguistically appropriate support to the Afghan Community. This position focuses on case management, resource coordination, and community integration to assist Afghans in achieving self-sufficiency and stability in the U.S.
Duties
- 1. Client Support & Case Management (40%)
- Conduct comprehensive intake assessments to identify client needs.
- Develop and implement Individualized Service Plans (ISP) for each client, covering areas such as housing, healthcare, education, and employment.
- Provide ongoing case management, including regular check-ins, home visits, and progress evaluations.
- Maintain detailed case notes and documentation in compliance with Office of Refugee Resettlement (ORR) guidelines and confidentiality regulations.
2. Community Integration & Advocacy (25%)
- Assist Afghan women in navigating essential services such as healthcare, legal aid, and public assistance.
- Provide referrals for English Language Training (ELT), workforce development programs, and childcare services.
- Address barriers to integration, including transportation, language, and cultural adaptation.
- Advocate for clients’ rights and access to services by coordinating with local agencies, healthcare providers, and legal resources.
3. Cultural Competency & Language Support (15%)
- Provide translation and interpretation support for Limited English Proficiency (LEP) clients.
- Develop and distribute culturally relevant materials (e.g., health awareness flyers, financial literacy guides, and domestic safety information).
- Conduct cultural orientation sessions on topics such as U.S. laws, social norms, and available community resources.
4. Program Coordination & Reporting (15%)
- Collaborate with community partners and local resettlement agencies to align services with existing refugee programs.
- Assist in data collection and reporting for Performance Progress Reports (PPR) as required by ECSH Program and ORR guidelines.
- Ensure compliance with federal guidelines on client eligibility, documentation, and safeguarding of personally identifiable information (PII).
5. Professional Development & Training (5%)
- Participate in trauma-informed care training and professional development workshops.
- Stay informed about policy changes, best practices, and resources affecting refugee resettlement and case management.
- Attend team meetings and contribute to program improvement strategies.
Experience
- Qualifications & Requirements:
- Experience: Minimum 2 years of experience in refugee services, case management, or social work, preferably with Afghan or Muslim communities.
- Language Proficiency: Fluency in Pashto and/or Dari is required; additional languages (Urdu, Arabic) are a plus.
- Cultural Competency: Strong understanding of Afghan cultural norms, gender roles, and challenges faced by newly arrived Afghan women.
- Technical Skills: Proficiency in Microsoft Office (Word, Excel) and case management software.
- Certifications: Trauma-informed care training is preferred.

- Education: Associate degree in Social Work, Human Services, Psychology, or a related field (bachelor’s preferred).
- Empathetic, patient, and committed to serving vulnerable populations.
- Strong communication, advocacy, and problem-solving skills.
