The Role:
We're seeking an experienced nurse to join our team as a Care Management Nurse (MDS). This role is ideal for a clinically focused professional who wants to remain involved in patient care without being a bedside nurse.
Responsibilities:
- Conduct comprehensive assessments and develop care plans for assigned residents.
- Work closely with the interdisciplinary team to ensure seamless care delivery.
- Maintain accurate records and reports, adhering to regulatory requirements.
- Collaborate with physicians, caregivers, and other stakeholders to address resident needs.
Requirements:
- Degree in Nursing (RN or LPN) with relevant experience.
- AANC certification (RAC-CT) is highly desirable.
- Familiarity with RAI process and PPS principles is strongly preferred.
- Proven track record in MDS nursing.
