Job Summary: We are seeking a skilled and experienced MDS Coordinator to join our team. The MDS Coordinator is responsible for coordinating and overseeing the Minimum Data Set (MDS) process in compliance with state and federal regulations. This position plays a crucial role in ensuring accurate and timely completion of resident assessments. Responsibilities: - Coordinate and complete the MDS assessment process for all residents in the nursing home facility - Ensure accurate coding of resident diagnoses, procedures, and treatments using ICD-10 coding standards - Collaborate with interdisciplinary team members to gather necessary information for assessments - Conduct comprehensive assessments of residents' physical, mental, and psychosocial well-being - Develop individualized care plans based on assessment findings and resident goals - Monitor and update care plans as needed to reflect changes in residents' conditions or goals - Provide education and training to staff regarding MDS documentation requirements and coding guidelines - Maintain accurate and up-to-date resident records in accordance with facility policies and procedures - Participate in quality improvement initiatives to enhance resident care outcomes Qualifications: - Valid nursing license (RN/LPN) required - Minimum of 2 years of experience in a nursing home or long-term care setting - Strong knowledge of MDS assessment process, Medicare/Medicaid regulations, and ICD-10 coding standards - Familiarity with memory care, family planning, and venipuncture procedures - Excellent organizational skills with attention to detail - Ability to effectively communicate with residents, families, and interdisciplinary team members - Proficiency in computer systems used for MDS documentation and reporting - Anatomy knowledge and understanding of medical terminology We offer competitive compensation packages including benefits such as health insurance, retirement plans, paid time off, professional development opportunities, and more.
