Child And Family Connections works with families in a 9 county area. We are autonomous and our goal is to serve children birth to 3 with possible developmental delays.

Full time position with regionally competitive benefits. Case Management/Service Coordination for the counties of McDonough, Hancock, Schuyler and Fulton to work with families who have infants and toddlers with possible developmental delays. Candidate must reside in the area of the counties they serve and be detailed, organized and willing to work some evenings if needed. Duties Include:
- Travel
- Home visits
- Coordinating services for families
- Documentation of State required paperwork
- Monthly staff meetings in Macomb, IL
- A Bachelor's degree or higher from an accredited college or university in Education, Social Work, Human Services, Behavioral Science or health related field.
- Official Transcript
- Eligible for Early Intervention Service Coordination Credential
- Proven successful experience working with children and families preferred.
